Bipolar Disorder Treatment

Bipolar disorder is often misunderstood and frequently misdiagnosed — commonly mistaken for depression for years before the pattern becomes clear. Accurate diagnosis and steady, well-explained treatment can make an enormous difference to stability and quality of life. This is an area of particular depth for Dr. Anindo Mitra (MBBS, MD Psychiatry, JIPMER), whose MD research examined clinical predictors of lithium response in bipolar I disorder. Care is available in-person in Gurugram and online across India.

Who this is for

This page is for you if you have experienced episodes of both low mood and periods of unusually elevated mood, energy, or activity — or if you have been treated for depression that never fully settled, with antidepressants sometimes making things worse or triggering agitation. It is also for families supporting someone with bipolar disorder.

Why accurate diagnosis matters so much

The difference between bipolar disorder and recurrent depression is one of the most important distinctions in psychiatry, because the treatments differ. Antidepressants used alone in bipolar disorder can be unhelpful or destabilising. Dr. Mitra takes a careful history — including any past episodes of elevated mood, reduced need for sleep, rapid thoughts, or impulsive behaviour that may not have seemed like a problem at the time — to get this right.

Treatment approach

Bipolar disorder is a long-term condition, and the aim is stability: fewer and milder episodes, and a life not dominated by illness. Treatment is individualised and explained fully:

  • Mood stabilisers are the foundation. Options include lithium, valproate, lamotrigine, carbamazepine, and certain atypical antipsychotics — each suited to different situations. See Mood Stabilizers in Bipolar Disorder.
  • Lithium, in particular, remains one of the most effective treatments and has strong evidence for preventing relapse and reducing suicide risk. Its benefits, monitoring, and safety are covered in depth on the Lithium Treatment for Bipolar Disorder page.
  • Managing episodes — treating acute depression or elevated mood safely, and adjusting treatment to prevent recurrence.
  • Psychoeducation and lifestyle. Understanding the illness, recognising early warning signs, protecting sleep, and building routine are powerful, evidence-based parts of staying well. Family sessions can help loved ones support recovery.

What to expect

With the right long-term treatment, many people with bipolar disorder achieve lasting stability and lead full lives. Treatment is reviewed regularly, doses are kept to the minimum effective level, and any changes are planned carefully — mood-stabilising medication is not stopped abruptly.

Frequently asked questions

How is bipolar disorder different from depression? Bipolar disorder involves episodes of elevated mood or energy (mania or hypomania) as well as depression, whereas depression involves low mood alone. The distinction is crucial because treatments differ significantly.

Do I need medication for life with bipolar disorder? Many people benefit from long-term mood-stabilising treatment to prevent relapse, but this is individual and reviewed regularly. Doses are kept to the minimum effective level, and treatment is never stopped abruptly.

Is lithium safe? Lithium is highly effective and, with proper monitoring of blood levels, kidney, and thyroid function, is safe for many people. Its benefits and safety are discussed in detail so you can make an informed choice.

Can antidepressants make bipolar disorder worse? Used alone, antidepressants can sometimes trigger agitation or elevated mood in bipolar disorder. This is why accurate diagnosis and mood-stabilising treatment come first.

Can bipolar disorder be managed online? Yes, stable bipolar disorder can be managed well with online follow-up, including monitoring, though some blood tests (for example for lithium) are done locally. Acute episodes may need in-person or emergency care.

Will I be able to live a normal life? Many people with bipolar disorder, once stable on the right treatment, lead full and productive lives. Stability and quality of life are the central goals of treatment.
